How the Abortion Pill Works: The 4-Step Process

We use a simplified 4-step abortion regimen with oral medications for your comfort and convenience.

Step 1: First Office Visit - Ultrasound and Medical Evaluation

Your first appointment is essential to determine if the abortion pill is right for you and safe for your situation.

Why the Ultrasound Is Critical:
Before we can provide the abortion pill, we must perform an ultrasound. This is not optional - it's a required safety step that:

- Accurately dates your pregnancy - Many women think they know how far along they are based on their last period, but this can be off by several weeks. The ultrasound gives us precise dating.

- Helps us recommend the best option - If you're close to 9 weeks, we'll discuss whether surgical abortion might be the better choice for you.

What Happens at Your First Visit:
- Pregnancy test to confirm pregnancy
- Ultrasound to determine gestational age (included in your fee)
- Medical history review
- Blood pressure and other vital signs
- Discussion of the complete process and what to expect
- All your questions answered
- If eligible, you'll take the first medication (mifepristone) in our office

This first medication blocks progesterone, the hormone needed for pregnancy to continue. You can eat and drink normally before this appointment - no fasting is required. Most women feel fine after taking this medication and can go about their normal activities.

The ultrasound and first visit typically take about 1-2 hours total.

Step 2: At Home (24-48 Hours Later) - Taking the Second Medication

24 to 48 hours after taking the first medication in our office, you will take the second set of medications (misoprostol) at home. We provide oral tablets that you place between your cheek and gum, instead of vaginal inserts, for your added comfort, safety, and convenience.

Preparing for Step 2:
- Plan to be at home in a comfortable, private place
- Have someone with you if possible for support and assistance
- Have sanitary pads ready (not tampons)
- Have pain medication available (ibuprofen works well - we'll give you specific recommendations)
- Have access to a phone to call us if needed
- Arrange to have the day free from work or other obligations

What the Second Medication Does:
The misoprostol causes your uterus to contract and expel the pregnancy. This is when you will experience cramping and bleeding, similar to a heavy menstrual period or miscarriage. This is completely normal and expected - it means the medication is working.

Timing Is Important:
Take the second medication exactly as instructed, 24-48 hours after the first medication. We will give you specific instructions about the exact timing and how to take the pills. Following these instructions carefully ensures the best chance of success.

Step 3: The Abortion Process at Home - What to Expect

After taking the second medication, the abortion process will begin. Here's what to expect:

When Symptoms Begin:
Cramping and bleeding typically begin within 1-4 hours after taking the second medication. Some women experience symptoms sooner, others may take longer. Everyone's experience is different.

What You Will Experience:
- Cramping - This can range from mild (like menstrual cramps) to strong. The cramping may come in waves. Pain medication can help manage this.

- Bleeding - You will have bleeding heavier than a normal period, often with clots. This is normal. You may pass the pregnancy tissue, which may look different from regular blood clots.

- Peak symptoms - The heaviest bleeding and strongest cramping usually occur 2-5 hours after taking the medication and last for several hours.

- Other symptoms - You may also experience nausea, vomiting, diarrhea, chills, low-grade fever (under 100.4°F), or fatigue. These are normal side effects.

The Timeline:
- Most women pass the pregnancy tissue within 4-6 hours of taking the second medication
- Heavy bleeding typically lasts several hours
- Lighter bleeding and spotting can continue for 2-4 weeks
- Cramping usually decreases significantly within 24 hours

Managing Your Comfort:
- Take ibuprofen (Advil, Motrin) as recommended - this helps with cramping
- Use a heating pad on your abdomen
- Rest and stay hydrated
- Eat light foods if you feel nauseated

Important - When to Call Us Immediately:
- If you do NOT experience any bleeding within 24 hours of taking the second medication - call us right away at 718-585-1010
- Soaking through 2 or more pads per hour for 2 hours or longer
- Severe pain that is not relieved by pain medication
- Fever over 100.4°F that lasts more than 24 hours
- Severe nausea and vomiting that prevents you from keeping down liquids

Step 4: Required Follow-Up Visit (7-14 Days After Your First Visit)

You MUST return for an in-person follow-up appointment 7-14 days after your first visit (when you took the first medication). This appointment is mandatory and critical to your care - it is not optional.

Why This Follow-Up Is Essential:
There is approximately a 3-7% chance that the abortion pill does not completely work, especially if you were close to 9 weeks pregnant when you started the process. The only way to know for certain that the abortion is complete is with an ultrasound.

If the abortion is incomplete and we don't catch it at your follow-up visit, you could experience:
- Continued pregnancy
- Infection
- Heavy bleeding
- Other complications

This follow-up visit protects your health and ensures the abortion was successful.

What Happens at Your Follow-Up Visit:
- Ultrasound to confirm the abortion is complete - this is the most important part
- Check your vital signs and overall recovery
- Discuss any symptoms or concerns you've had
- Answer any questions
- Discuss birth control options to prevent future unintended pregnancy
- Provide birth control if desired (IUDs and Nexplanon can be inserted at this visit)

The follow-up visit typically takes 30-60 minutes.

What If the Abortion Is Incomplete?
If the ultrasound shows the abortion pill did not completely work, you will need a surgical abortion procedure (D&C) to complete the process. This is a quick, safe procedure done in our office with general anesthesia, and you can go home the same day.

The fee for a D&C following an incomplete medication abortion is $600 for self-pay patients. Most insurance plans and Medicaid cover this as part of your abortion care. We will verify your coverage before proceeding. If you need this follow-up procedure, we will work with you to ensure you can complete your care safely.

Important - Scheduling and Attending Your Follow-Up:
We will schedule your follow-up appointment for 7-14 days after your first visit before you leave. Please keep this appointment - it's essential for your health and safety.

If you miss your scheduled follow-up appointment, we will attempt to contact you by phone, text, and email to reschedule. However, if we are unable to reach you after two attempts, we will be required to send you a letter of dismissal from our practice.

We cannot be held responsible for any medical complications that may arise if you do not complete your required follow-up care. Your health and safety depend on confirming that the abortion is complete.

Recovery and Aftercare Instructions

When Can I Return to Normal Activities?

Most women feel well enough to return to work, school, and light daily activities the day after taking the second medication, if they feel up to it. Listen to your body and rest as needed.

Avoid strenuous exercise, heavy lifting, or intense workouts for at least one week. Light activities like walking are fine when you feel ready. If you notice increased bleeding or cramping with any activity, slow down and rest.

Using Pads vs. Tampons

We recommend using sanitary pads (not tampons) during the first few days of heavy bleeding. This allows you to monitor your bleeding more easily and ensures you can pass tissue and clots safely.

Once your bleeding becomes lighter (usually after a few days), you may switch to tampons or menstrual cups if you prefer. If bleeding increases after switching, go back to using pads.

Bathing and Swimming

You may shower normally. However, avoid baths, hot tubs, and swimming pools for at least one week to reduce the risk of infection while your cervix is still slightly open.

When Can I Have Sex Again?

You can resume sexual activity when you feel physically and emotionally ready, typically after your bleeding has decreased. However, it's important to know that you can get pregnant very quickly after an abortion - as soon as two weeks later, even before your first period returns.

If you don't want to become pregnant, start using birth control immediately. We can provide IUDs, Nexplanon, or other contraception methods at your follow-up visit.

When Will My Period Return?

Your next menstrual period should begin 4-6 weeks after your abortion. Your first few periods may be irregular as your hormones adjust, which is normal. If you don't get your period within 6 weeks, call us at 718-585-1010.

The effectiveness decreases the further along you are:
- At 7 weeks or less: 97-98% effective
- At 8-9 weeks: 93-95% effective

This is why the ultrasound is so important - it ensures you're within the optimal timeframe for the medication to work. If you're close to or beyond 9 weeks, surgical abortion becomes the better option with over 99% effectiveness.
